Scholarship Forms

Undergraduate Scholarship on-line application will be available December 15, 2011 and must be completed by March 1, 2012. Late or incomplete applications will not be accepted. To apply for scholarships, go to https://scholarships.wisc.edu/ScholarshipApp

Graduate Scholarship Application - Students will complete and submit the application online. Application is available April 1, 2012 through June 1, 2012. Please note: Students will use their UW-Madison NETID and password to login to the scholarship application.

Funding Forms

NAO Student Support Application (pdf) - applications are accepted twice a year, March 1 and November 1, from undergraduate and graduate nursing students currently enrolled at the UW Madison to request support for special individual projects or training.

Travel Funding Form - Contact Carol Aspinwall at caaspinwall@wisc.edu for a electronic Travel Funding Form. Students return the completed travel request form to the Academic Programs Office (K6/146) or Carol Aspinwall (K6/136) as soon as they are notified their abstracts have been accepted. A copy of the abstract and conference flyer should be included with the travel request and must have the signature of your nursing advisor.
