Demographic Form
This form is used by current or newly admitted students to provide advisors with demographics information, educational background, and nursing experience. It is important that you use the email address assigned to you by your home institution. This will ensure prompt and accurate communication.
Commitment Form
Determine which courses you wish to enroll in for the upcoming semester. A commitment form submitted by you and approved by your advisor holds a spot for you in class(es); however, once your commitment form is approved by your advisor you must officially register for the course at your home institution.

The BSN@Home program is provided through the University of Wisconsin System,
administered by Collaborative Nursing Program consisting of the following schools:
UW-Eau Claire, UW-Green Bay, UW-Madison, UW-Milwaukee and UW-Oshkosh.
