Demographic Form
This form is used to indicate your chosen home institution, and provides the institution's advisors with your educational background and nursing experience. It is important for students to input their institutional email once assigned, ex. johndoe@wisc.edu. This will ensure prompt and accurate communication.
Commitment Form
Determine which courses you wish to enroll in for the upcoming semester. A commitment form submitted by you and approved by your advisor holds a spot for you in class(es); however, once your commitment form is approved by your advisor you must officially register for the course at your home institution.
The BSN@Home program is provided through the University of Wisconsin System,
administered by Collaborative Nursing Program consisting of the following schools:
UW-Eau Claire, UW-Green Bay, UW-Madison, UW-Milwaukee and UW-Oshkosh.
