Commitment forms will not be accepted after Friday,
August 15th, for
the Fall 2008 session.
Classes may close well before this closing date. Commitment forms are approved on a first come, first served basis. Once your form is approved, you will still need to register for your classes at your home institution. Registration methods vary for each home institution.
It is important for students to input their institution email once assigned, ex. johndoe@wisc.edu. This will ensure prompt and accurate communication.
Contact your advisor for further information. Students must have access to a computer, Internet connection and email address prior to August 15, 2008.
Commitment Form
Determine which courses you will be attending for the upcoming semester. A commitment form submitted by you and approved by your advisor holds a spot for you in class(es).
The Fall 2008 commitment form is closed because all courses are full. Please contact your advisor about being placed on a waiting list.
The BSN@Home program is provided through the University of Wisconsin System,
administered by Collaborative Nursing Program consisting of the following schools:
UW-Eau Claire, UW-Green Bay, UW-Madison, UW-Milwaukee and UW-Oshkosh.
